Reporting serious incidents

Abingdon Concert Band (hereafter referred to as “the Band”) will adhere to the Charity Commission’s guidance on how to report a serious incident1

The guidance outlines the type of incident that should be reported. In the context of the Band, this includes:

  • Actual or risk of harm to the people who come into contact with the Band
  • Actual or risk of loss of the Band’s money or assets 
  • Actual or risk of harm to the work or reputation of the Band

Any Band member can identify a situation they believe could be a Serious Incident and alert the Trustees to this. Any Trustee can identify a situation they believe could be a Serious Incident and alert the other Trustees to this. 

The Trustees will follow the Charity Commission’s guidance to determine if the incident requires reporting. Should the incident involve one or more Trustees, those Trustees should be excluded from the process to determine whether the incident requires reporting. 

If the incident requires reporting to the Charity Commission as a Serious Incident the Trustees are responsible for taking action in accordance with the Charity Commission’s guidance, noting that reporting a Serious Incident or potential Serious Incident to the Charity Commission does not negate the need to alert all other applicable authorities to the incident. 

If the incident does not require reporting to the Charity Commission as a Serious Incident the Trustees should inform the Band member or Trustee who identified the issue of their decision, document the reason for the decision in writing and file this on the Band’s GoogleDrive. The Trustees should still review the incident in the context of the Band’s other policies and identify if any other remedial action is required.
